Description
Iredell Health System will be providing complimentary sports physicals for student athletes at North Iredell High School and rising freshmen attending North in the fall of 2023.
- Please ensure the examination form with parental consent has been completed and signed for the physical. Bring the completed parental and medical history forms to the screening day.
- No insurance information will be required or recorded.
- Physicals will be conducted by providers from Iredell Physician Network.
- $10 donation to the North Iredell Booster Club is requested for participation.
