Here is how to fill it out - First Column (Enter Name of Primary IBO i.e. Fred Smith) Second Column (Enter total number of people including Primary IBO i.e. Fred Smith, his wife and 3 kids would equal 5)
We are not accepting any cash, and you will need to have your ticket scanned on your mobile device to get entry. We need to cut off sales at noon on 12.31.19 so that we can order the correct amount of pizza.
